Please remember that your insurance is a contract between you and your insurance company, and some plans may have limited benefits. Please check with your insurance company to see if pre-authorization is required. This is not necessary if you are covered by Medicare.
It is important for us to have your most recent and correct insurance information, so we ask for a copy of your ID card at each visit.
Payment / Billing
Patients are responsible for co-payments and remaining deductibles before services are rendered. Our practice accepts cash, check, and most major credit cards.
We do not want anyone to be denied necessary medical care because of an inability to pay. If you have difficulties understanding or paying your bill, we encourage you to discuss your problem with the business office staff.
